Where form responses show up
Form and survey responses show up in three places: the Submissions screens under Sites, the contact record's Activity section, and the contact's Contact Details page.
Review form submissions
The main place to review form answers is Sites > Forms > Submissions. From this screen you can filter form submissions, search them, and view the information provided in a particular submission.
From the left-hand menu, go to Sites.
Open the Forms tab and click Submissions at the top of the screen.
Review the list, then select an entry to see the details of that submission.
Each list entry shows the submitted values, such as Full Name, Email, Phone, Subject, Organization, Postal Code, and a timestamp. Selecting an entry shows the information supplied on that submission, plus page detail such as URL, medium, source, referrer, and timezone.
Search and filter submissions
The Submissions screen lets you filter form submissions and search them from one page. The search box accepts "Search by name, email, phone and contact id".
Answers on the contact record
Submitted answers also live on the contact's record: custom fields used on your forms appear on the contact's Contact Details page, and every submission is recorded in the Activity section.
From the left-hand menu, go to Contacts, then search for or click the contact.

To review the submission itself, open the Activity section in the right column of the contact record, using the icon that looks like a clock. Selecting a submission shows the information the contact submitted. In most cases, fields collected by the form are also written back to the contact's record. Survey submissions
Review survey responses from Sites > Surveys > Submissions.
From the left-hand menu, go to Sites.
Select the Surveys tab and click Submissions.
Review the list of submissions, which includes timestamps and answers.
Click an individual entry to see all the responses.
