Forms, surveys, and quizzes: what to use when
JourneyCARE Forms, Surveys, and Quizzes all collect information directly into your contact database. They work the same way behind the scenes; the difference is the experience you create: Forms show all questions on one page, Surveys ask one question at a time, and Quizzes score responses and show personalized results.
Forms
Forms are the most commonly used option and display all questions on a single page, making them ideal for collecting multiple pieces of information at once. Use a form when you want to collect information such as membership applications, event registrations, committee interest forms, contact requests, sponsorship applications, or general information updates.
Surveys
Surveys use the same field types and functionality as Forms but present one question at a time. This format creates a more conversational experience and works well for member satisfaction surveys, event evaluations, feedback forms, and opinion polls. The primary difference between a Form and a Survey is the presentation, not the information that is collected.
Quizzes
Quizzes are designed for assessments and scored experiences. In addition to collecting responses, quizzes can calculate scores, display personalized results, and guide users through a series of questions. Organizations often use quizzes for educational assessments, knowledge checks, or interactive lead generation.
How they work together
Although Forms, Surveys, and Quizzes look different to the person completing them, each can collect information from a user, update their contact record, trigger workflows and automations, and store responses for future reference. Because they all connect to the same contact database, you can use the collected information throughout JourneyCARE.

How forms connect to contacts
Every form submission is connected to a contact record. When someone submits a form, their responses are linked to their contact record, the submission appears in their activity history, contact fields can be updated automatically, and workflows and automations can begin immediately based on the submission. Forms don't simply collect information; they become part of your organization's complete member experience.

Use Forms for most day-to-day data collection.
Use Surveys when you want a more guided question-and-answer experience.
Use Quizzes when responses need to be scored or evaluated.
Before creating new fields on a form, check whether the field already exists in your database to avoid creating duplicate data.
