Create a form
Create a form from Sites > Forms: click Create New Form, give it a name, and the Form Builder opens with a drag-and-drop canvas where you add fields, products, conditional logic, and other elements. Forms collect information from members, event attendees, applicants, sponsors, volunteers, and anyone else interacting with your organization, and they can collect payments in the same submission.
Before you begin
Plan the form before you build it. Decide who will complete the form, what information you need to collect, whether submitted information should update an existing contact record, whether the form will collect payments, and what should happen after the form is submitted. Planning these items first saves time later.
Create a new form
Open the Forms area, create the form, and the Form Builder opens to the form canvas.
Go to Sites > Forms.
Click Create New Form.
Give your form a descriptive name.
The Form Builder opens.
You'll now be working on the Form Canvas.
Understand the Form Canvas
The canvas is your workspace for building the form. From here you can add new fields, rearrange fields, create sections, add payment options, configure conditional logic, and customize the appearance of the form. It is a visual layout editor: drag elements into the form and arrange them in the order you want users to complete them.
Add fields with Quick Add
Quick Add is the fastest way to build a form. It lists common elements — text fields, email, phone, dropdowns, checkboxes, date pickers, file uploads, products, sections, paragraph text, and buttons — and you insert an element by clicking or dragging it onto the canvas.
Understand Object Fields
Many fields in JourneyCARE connect directly to information stored on the contact record. These are called Object Fields, and examples include First Name, Last Name, Email Address, Phone Number, Company, and Address. When someone completes the form, these fields automatically update their contact record.
Whenever possible, use existing Object Fields instead of creating custom fields for common information.
Save and test your form
JourneyCARE automatically saves many changes while you're working, but it's still a good habit to click Save periodically. Before publishing your form, review all fields, test required fields, verify any payment settings, confirm automations connected to the form, and submit a test entry. Testing before sharing your form helps catch small issues before your members do.
Common mistakes
Avoid the most common form-building errors:
Creating custom fields instead of using existing contact fields.
Forgetting to save after major edits.
Publishing without testing.
Collecting information that isn't actually needed.
Making every field required.
