Paperform App (for anonymous form submissions)

Sometimes organizations need to create a form that should not be tied to the contact record. For example, you may have a survey or evaluation form that people would be reluctant to submit if they were identified.

For these special situations, we have integrated the Paperform app into our Premium Website Editor. Simply contact our Help Desk at [email protected] and we can add that app to your site.

Create a form in Paperform

Once the app is added to your site, you will build forms inside the app. When you open the app, you land on the Paperform Dashboard. Click Create a form, then choose one of four ways to build:

• AI form creator — describe the form you want and let Paperform draft it.

• PDF or image import — turn an existing PDF or images of a form into a Paperform.

• Template — start from one of over 30,000 pre-made templates.

• Start from scratch — open the editor with a blank form and build it yourself.

Create a form with AI

In the “What form are you looking to create?” box, describe the form’s purpose, the outcome you want, and some or all of the questions you want to ask. Shortcut buttons below the box offer ready-made prompts, and you can attach an existing form with Attach PDF or images of a form to import.

1. Enter your prompt and, if you have one, attach a PDF or images of an existing form.

2. Click Create with AI.

3. Answer any extra questions Paperform asks to refine the form’s questions.

4. Review the draft the AI generates, then give feedback or request changes.

5. When you’re happy with it, click Continue to Editor to open the Form Editor and add finishing touches.

Start from a template

In the template area, search or browse the library of over 30,000 templates. Click a form to preview it, then click Use this template to begin. Paperform’s templates page lets you filter by categories, industries, and roles.

Start from scratch

Select Start from scratch to open the editor with a blank form, then add and edit your questions and other content manually.

Manage forms from the Dashboard

From the Dashboard, use the three-dot icon on a form to Make a copy or Delete it. Rename a form by clicking its name in the Dashboard, or in the editor under Configure > Details.
