Customize the Add Contact form
Admins customize the Add Contact form — the slide-out sidebar where staff create contacts — from Contacts > Smart Lists: open it with + Add Contact, click Customize Form, arrange the fields, then save the layout for your team.
About the Add Contact form
The upgraded Add Contact experience replaces the classic pop-up with a modern slide-out sidebar and adds admin controls for which fields appear and in what order.
The slide-out modal supports standard and contact-level custom fields, drag-and-drop field ordering, field-level Required toggles, and Preview. It also protects against losing work: Save creates the contact, Save & Add Another creates the contact and opens a blank form for the next entry, and closing the modal with unsaved changes prompts you to confirm before discarding them.
The upgraded Add Contact form is currently available in Labs. If you don't see the slide-out sidebar, ask an admin to enable the upgrade at Settings > Labs.
Who can customize the form
Admin-level users in the organization can customize the form layout, as set in Roles & Permissions.
Any user with access to the Contacts area can open Add Contact and use the configured form to create contacts, but only admin-level users can change the layout.
Customize the form
Build the layout your team will use from the same place they add contacts: launch the modal, then open the customization view.
Go to Contacts > Smart Lists.
Click + Add Contact to launch the modal.
Click Customize Form.
Add or remove standard and contact-level custom fields. Contact-level custom fields are managed in Settings > Custom Fields; learn how to create custom contact fields.
Drag fields to reorder them.
Turn on Required for any field that must be completed before saving the contact form.
Use Preview to validate the layout.
Save the layout for your team.
Layout changes appear immediately for new sessions; users who already have the page open may need to refresh the Contacts page to load the latest configuration.
Optionally, pair the form with automation: a Contact Created trigger in a Workflow can send welcome messages, assign an owner, or apply tags to new contacts automatically. See Working with Contact Tags for how tags power those automations.
The Manage Smart Lists and Restore options moved from the page header into the three-dot (⋮) actions menu beside the Add Contact button. Smart List permissions did not change — only the location of those menu options.
Add a contact with the form
Complete the required fields, then save the contact with one of three actions.
Save — creates the contact. Each saved contact gets a Contact Record that tracks its activity and communication history.
Save & Add Another — creates the contact and reopens a blank form for the next entry.
Cancel — exits the modal without saving.
If you try to close the modal without saving changes, an unsaved-changes prompt asks you to confirm before the form closes.
Required rules in the Add Contact modal apply to manual entry only; CSV imports and API requests have their own validations.

Prevent duplicate contacts
Set deduplication preferences to control how JourneyCARE matches duplicate contacts before they pile up.
Configure Settings > Business Profile > Contact Deduplication Preferences to choose primary and secondary match fields, such as Email or Phone.
CSV imports always check email and phone and merge matching contacts, so duplicates cannot be created via CSV import. If duplicates still slip in from forms or integrations, you can merge up to 10 contacts and choose a master record.
